2. Removal of Content

3. Counter Notification

- Your name, address, and phone number.
- The URL of the content that was removed.
- A statement under penalty of perjury that you believe the content was removed by mistake.
- A statement that you consent to the jurisdiction of the court in your area.
- Your signature (electronic is acceptable).
